DODGEBALL TOURNAMENT REGISTRATION

Please complete the below form to register your team for the Dodgeball Tournament to be held on Sunday, February 27th at 1:00PM in the Scranton Gym.  Teams will consist of 6 players with one member being designated as the team Captain.  The Captain will be notified of any changes in tournament details.  The team Captain will be responsible for notifying the other team members of any changes.  All players on a team must fit within the designated age group.  The age groups are:  4th Grade & Under, 5th thru 7th Grade and 8th Grade and Up.

PLEASE NOTE:   All players MUST complete a waiver form.  Printable waivers are available here or at the door the day of the tournament. Those under age 18 must have a waiver signed by a parent or legal guardian.  Those without signed waivers will not be allowed to participate.  

Admission is $1 per person for all players and spectators. Payment will be due at the door.  This tournament is for fun and entertainment purposes only.  No prizes will be awarded.
